Michigan State University plan
This plan covers MSU employees who don't have Medicare as their primary coverage.
The Michigan State University plan doesn't require global referrals or plan notification to process and pay claims; however, some services require prior authorization regardless of whether they are performed at the in-network or out-of-network benefit level. In addition, certain preventive services, such as physical exams and immunizations, are covered only when provided in network.

MSU Plan 65 (for MSU employees)
This plan covers MSU employees who have Medicare as their primary coverage. Global referrals and plan notification aren't required for claims payment for members with this coverage. In addition, prior authorization is not required, except for:
- Home infusion services not covered by Medicare
- Inpatient care when Medicare days are exhausted
